Sermorelin is a synthetic 29-amino-acid peptide corresponding to the biologically active N-terminal fragment of human growth hormone-releasing hormone, GHRH(1-29). The truncated sequence retains full receptor activity relative to the 44-residue native hormone.

Sermorelin binds the GHRH receptor, a class B GPCR expressed on anterior pituitary somatotrophs, activating Gs-coupled adenylate cyclase and raising cAMP, which in turn drives growth hormone transcription and release in a pulsatile pattern.
It is frequently compared with tesamorelin, a stabilized GHRH analog, and with ghrelin-receptor secretagogues such as ipamorelin, which act through a distinct receptor and are often co-administered in experimental designs to examine pathway additivity.
